Supported Form Tools

2/26/25 NOTE: The University of Tennessee will be moving away from Google to Microsoft as the university-wide platform for collaboration and productivity tools. This change is slated to be completed by January, 2026. This means that Google Forms will no longer be a supported form tool.

The following are form tools that are supported by UTIA Information Technology Services (ITS) and best practices for using each form tool:

  • Qualtrics
    • Best for
      • Forms with file uploads
      • Registration Forms
      • Scholarship Applications
    • Best to use with Personally Identifiable Information (PII) and Family Educational Rights and Privacy Act (FERPA) data
    • Use departmental NetID to create and maintain form
  • Microsoft PowerApps
    • Best for:
      • Internal/External Forms
      • Forms that need editing and an admin panel
      • Forms with auto-completion of some fields, such as student data
    • To have a PowerApps form created for your department/unit, please submit a project request for the ITS team.
  • Microsoft Forms
    • Best for:
      • Simple surveys
      • Registration forms
      • County Office Site Visits without payment
      • Can be used for external forms – settings must be changed to allow this
      • Automation can be added to Micrsoft Forms
    • Use departmental NetID to create and maintain form
    • To discuss options for Microsoft Forms and best practices, please submit a project request for the ITS team.
  • Formidable
    • Best for simple contact us forms
    • Must be used with certain settings to keep PII data safe
      • Check setting to not store data in WordPress and set up notification to email form responses
    • To have a Formidable form created for your department/unit, please submit a project request for the ITS team.
    • For county offices that are needing a Formidable form for their website, please contact your regional trainer.

The form tools have been checked to ensure they meet the following standards:

  • Accessibility
  • FERPA – Family Educational Rights and Privacy Act that covers student data
  • PII – Personal Identifiable Information that covers personal data such as name, email address and phone number
  • HIPPA – Health Insurance Portability and Accountability Act of 1996 that covers patient health information.
